Insurance & Billing

Please bring your insurance card with you at the time of your visit so we may verify your participation. We accept most types of insurance and Medicare. If your insurance covers office visits, we will complete the necessary forms and bill the insurance company for you.

We offer competitive fees for those patients choosing to self-pay.

Co-payments must be paid at the time of service. We accept cash, personal checks, and credit cards.

If your medical bills are to be covered under Worker's Compensation or auto insurance please inform our office at the time of your visit.

If we refer you to a specialist, it is your responsibility to obtain the appropriate referral authorization from our office before seeing the specialist. See the Request a Referral form at our Forms Page.
